WHERE TO COMPLETE: The Hub

A parent/caregiver's personalized Contact Information Update form is used for Penn Charter's family directory. The form will appear when you log in to The Hub ("You have: Forms to Review") and is due Friday, May 24.

Once the form is complete, it can be reviewed by selecting "Show completed forms" found in Files and Forms under the parent or caregiver's name at the top of the Hub page.

WHO: Upper and Middle School Parents and Caregivers
WHERE TO COMPLETE: The Hub

All parents of Middle and Upper School students are requested to complete a 1:1 Laptop Program form in the Hub by Friday, May 24. Students may either bring to school a family-owned laptop that meets program requirements or participate in the Penn Charter laptop-loan program. The cost of the laptop loan is $225 and is billed in November.

If your child does not own a laptop that meets the requirements, and if the cost of the loan is a financial hardship for your family, you may complete a Grace Fund request. Contact PC Director of Technology Infrastructure and Planning Dan Volpe with questions.

WHERE TO COMPLETE: The Hub/Magnus Health App

Visit your Health Portal in the Hub by going to the Resources tab and clicking on PC Health Portal. Here you will be able to print any health forms that require a doctor's signature. Completed health forms are due July 31. You will need your health insurance information. Forms cannot be mailed or faxed to PC or Magnus. Parents and caregivers have two ways to submit health forms electronically:

1. Upload through the Health Portal in the Hub OR
2. Upload through the Magnus Health app. Take a photo of the health forms, then upload from your phone. View instructions and video if you need assistance.
